New Video – Speak Up: Making Dialysis Safer for Patients

The Coalition and the American Association of Kidney Patients have released a new video for patients on dialysis and dialysis staff about the importance of speaking up to prevent infections and improve patient safety. Infections are serious and sometimes life-threatening, but together we can do something to stop them.
The video follows the journey of two patients on dialysis who feel empowered to speak up and share concerns about their treatment, in order to prevent infections. The video also highlights ways dialysis staff can create an environment where patients feel safe speaking up.
This video is open access. You are free to use it in any communication channels. Patients are encouraged to post the video on social media and share with family and friends. Clinics are encouraged to show the video during dialysis treatments, play it in waiting rooms, and post links within patient and staff educational materials.

What to expect for the COHFI Launch Monday February 3rd
To: All Colorado Health Facilities Web Portal users

Our new Colorado Health Facilities Interactive (COHFI) system is launching this Monday, February 3rd. If you are a web portal admin, then on Monday morning you will receive an email from us welcoming you to the new COHFI system. The email will include your username and a link to COHFI. If you are not able to sign in that day you will be able to reset your password at any time afterwards.
Once you have signed in you will be able to view the facilities/agencies you are associated with and you will be able to create additional users for each facility.
For the month of February, access to the system may be limited in the evenings after 5pm as that is when we will be applying any needed fixes to issues that were discovered.
We have previously provided detailed guidance on what to expect for Occurrence Reporting, License applications, Plan of Corrections, Flu Vaccination Reporting and all of the current functions of the web portal. Those communications can be found on our blog along with links to our training videos. Once you have signed into COHFI you will have access to the User Manual which will also have the training videos and step-by-step guidance on how to complete required tasks.
Lastly, if you do encounter an issue with the system, we want to know about it. We have a section in the User Manual called Known Issues/Report a Bug where you can see the list of issues we know we are launching with and you can let us know about new issues you may be having. You can always call us as well, we are here to help. We know this system is not going to be perfect on Monday, but we are confident that with your support, we will be able to respond to your issues in a timely manner.

Flu Vaccination, Census, and other functions transition to COHFI 2/3/2020

There are several functions in the current Colorado Health Facilities Web Portal beyond Licensing, Plan of Correction, and Occurrence Reporting.
Nursing Facility Census:
We are working with our Medicare and Medicaid partners to retrieve the census and payer information from the MDS submissions you are already completing. This change is not complete but once it is finalized you will no longer need to submit quarterly census information. Until then, Census submissions are suspended.
Flu Vaccination Reporting:
The current Colorado Health Facilities Web Portal will continue to remain active through the end of the flu vaccination reporting season. Please continue submitting your flu vaccination reports as normal and we share how to submit future flu vaccination reports as we get closer to the next reporting season.
Electronic Letter of Intent and Facility Contact information have been incorporated into the Licensing and Facility functions in COHFI.
We are working hard to make this transition as seamless as possible, while acknowledging that it will be difficult for all involved. In the time leading up to the go live date, there will be continued messaging and training materials made available.

As we near the go-live date for the new Colorado Health Facilities Interactive (COHFI) application, February 3rd 2020, there will be some important dates to keep in mind regarding the Survey Tracking module which will be the part of the system that will allow you to submit plans of correction (PoC's).
Beginning 01/29/2020, all new deficiency and sample lists will continue to be sent to you via email however, those deficiency lists will not be uploaded to the current HFEMSD Web Portal application. The deficiency lists will instead be uploaded to the new COHFI Survey Tracking module following go-live on 02/03/2020.
On 02/03/2020 and 02/04/2020, all providers that were issued their deficiency list prior to 01/29/2020 but have not yet submitted an approved PoC, will be transitioned into the new COHFI Survey Tracking module, no matter what stage of the PoC process you are in, whether that be not yet submitted, submitted but not yet reviewed or reviewed and rejected for additional information. Those PoC's will then be finalized in the new application. The current HFEMSD Web Portal application will continue to be accessible but PoC's will no longer be reviewed through it beginning on 02/03/2020.
All providers who have submitted an approved PoC prior to 02/03/2020, but have not yet received a revisit to determine compliance, will finish out the revisit process outside of the new COHFI Survey Tracking module. This means that you will continue to submit your revisit documentation via email or fax and will not have the option to submit those documents through the module.
We understand that the transition into the new COHFI application will not be seamless. Therefore we want all providers to know that while we will still be maintaining our current expectations in terms of submission deadlines for PoC's, that we will continue to remain flexible and willing to approve extensions to those deadlines for any providers that may need them. If you are one of the providers who is or will be going through the PoC process during this transition, you may reach out to Jason Bohl at 303-692-6221 or by email at jason.bohl@state.co.us, to request an extension or if you have any technical difficulties using the new COHFI Survey Tracking Module. As discussed in a previous message from Randy Kuykendall, the Colorado Health Facilities Interactive (COHFI) system will go live on February 3, 2020. To implement the new COHFI system, there are 2 important dates to know for occurrence reporting.
January 22, 2020 - Occurrence summaries will be sent to providers for review. These summaries will be the last batch sent out from the current occurrence system. This will allow providers time to review the occurrence summaries in the current system before the go live date of COHFI on 2/3/20.
February 3, 2020 - COHFI goes live. All new occurrences must be submitted through the COHFI system on this date. Occurrences that have been in process will be moved by CDPHE staff to the COHFI system for completion. This process may take up to 4 weeks and requires that the occurrence will change identifying numbers.
CDPHE strives to make this transition as seamless as possible, while acknowledging that it will be difficult for all involved. In the time leading up to the go live date, there will be continued messaging and training materials made available. Although the reporting system is new, there are no changes in the reporting requirements. During this transitional phase (January 17th through February 2, 2020), facilities/agencies will be unable to submit electronic applications to HFEMSD for renewal licensing.

License Renewal Application Already Submitted, License Issued On Or After February 3, 2020: For licenses that are issued on or after February 3, 2020, the renewal process will need to be completed using the new renewal application in the COHFI system.
No License Renewal Application Submitted, License Expires On Or Before February 3, 2020: While the department will not require you to complete your license renewal application until the new COHFI system is available on February 3, 2020, the applicable license fees are still due prior to the expiration date of the license. A real case with evidence from multiple sources will be presented and analyzed. Participants will actively conduct their own RCA using a fishbone diagram from the case presented. Additionally, anticipated and a hierarchy of interventions are reviewed while utilizing an impact & effort matrix.
We will discuss and reflect on Quality Management Plan concepts and methodology as applied to home health (HH) licensure and federal regulations. Participants will learn which data and how it is gathered and becomes information to inform QMP functions within a home health agency. In addition, we will discuss real case elements and findings to pull all of the concepts together.

Updated Chapter 2 General Licensing Regulations

Pursuant to Section 24-4-103.3, C.R.S., and Department policy, the Department must review its rules every five to seven years to ensure the rules continue to be efficient, effective, and essential. Accordingly, in 2018 and 2019 the Department reviewed the existing 6 CCR 1011-1, Standards for Hospitals and Health Facilities, Chapter 2 General Licensure Standards. The proposed draft regulations were adopted by the State Board of Health in November 2019 and were made effective on January 14, 2020. They can be found on the Colorado Secretary of States webpage or at this link: Secretary of State, Code of Colorado Regulations .
Summary of Changes:
During the course of the rule review, the Department identified areas where technical changes should be made and where substantive regulatory additions were necessary. Areas of technical changes included the consolidation and movement of the definitions, which were previously throughout the Chapter, to Part 1. This change ensured consistency in the use of terms throughout the Chapter and enables readers to more easily find definitions. Parts within the Chapter were also relocated to provide better clarity, and duplication of information was removed as much as possible. Terminology was also updated to remove the total focus on health care facilities and services to recognize that not all facilities and agencies licensed by the Department are medical in nature.
While the statutory authority for licensing facilities and agencies has not been significantly changed in a number of years, statutes that inform Chapter 2 have been. Therefore, Part 8, Protection of Clients from Involuntary Restraint or Seclusion, was updated to align with changes made to Section 26-20-108, C.R.S. Nomenclature was also changed to align with Section 25-3-607, C.R.S, from "hospital-acquired" infection reporting to "health-care-acquired" infection reporting.
The major substantive change to the Chapter is the addition of Part 3, General Building and Fire Safety Provisions. The changes update the Facilities Guidelines Institute (FGI) standard from the 2010 edition to the 2018 edition, which previously existed within each of the individual licensing chapters, and also create a process, in regulation for the first time, as to how the FGI compliance review will take place. In placing the incorporation of FGI in Chapter 2, the Department also made conforming amendments to all other chapters within 6 CCR 1011-1 to remove the FGI references, expect for Chapter 7 Assisted Living Residences. Chapter 7 had already been updated to reference the 2018 edition of FGI.
Other substantive changes to the Chapter include the removal of language that an applicant shall pay a 100% late fee if a license renewal is not submitted 30 days in advance, which is now replaced with a tiered late fee based on receipt of the renewal application after expiration of a license; clarification as to the time period over which a transfer of ownership that equals 50% interest takes place; clarification as to when the Department considers that a non-profit transfer of ownership has taken place; and a process for facilities and agencies to cease operations.
Due to the nature of Chapter 2 being a reference for all other chapters within 6 CCR 1011-1, the Department also completed any necessary conforming amendments to those chapters. The conforming amendments removed the references to FGI as mentioned above, as well as updated references to Chapter 2 to accurately reflect the proposed structure.

Colorado Health Facilities Interactive Going Live

The Health Facilities and EMS Division are pleased to announce that after 2 years of planning and effort, the Colorado Health Facilities Interactive (COHFI) state facility licensing system is nearing completion.
The "go live" date for this transition is February 3, 2020. This actually completes phase 1 of the system's development and over time, additional capabilities and features will be added. Over the coming weeks the division will be communicating details regarding the transition to the new system, and what to expect regarding Licensing, Plans of Correction, Flu Vaccination reporting and Occurrence reporting. We recognize this is a significant change to our regulated community and as with most software system changes of this nature, COHFI will likely have some bugs as it comes on-line. However, the division is confident that we can work through any glitches with everyone's support and open communication. It is extremely important that any system issues that your facility may encounter be communicated to us in a timely manner.
We will be providing training materials, manuals, etc. and are planning to be as responsive as possible to assist you during the transition to the new system. Please know that this is not the final version of this system. It will evolve based on legislative, regulatory and provider community needs in the months to come. At roll-out we will have processes in place to receive your feedback on the system and will be working with our development partners to resolve issues as quickly as possible.
