Changes to the Colorado HAI Reportable Conditions & NHSN Annual Data Freeze

 

To: Infection Preventionists and Facility Administrators From: CDPHE Healthcare-Associated Infections and Antimicrobial Resistance (HAI/AR) Program  Changes to Colorado HAI Reportable Conditions On Jan. 27, 2026, the Colorado Healthcare-Associated Infections (HAI) Advisory Committee voted to remove the following conditions from the HAI reportable conditions list in Colorado: Surgical site infections (SSIs) following outpatient colon surgery in acute care hospitals (ACHs) Central line-associated bloodstream infections (CLABSIs) in critical access hospitals (CAHs) SSIs following the following procedures in CAHs: Inpatient and outpatient breast surgery Outpatient colon surgery Inpatient and outpatient hysterectomy The committee removed these conditions because the low volume of HAI data made the results statistically unactionable and required suppression in the HAI Annual Report. Effective immediately, the conditions above are no longer reportable in Colorado. Please review the updated list of HAI reportable conditions and confer rights to the appropriate CDPHE National Healthcare Safety Network group for these conditions to ensure you continue to meet all other Colorado HAI reporting requirements. Changes to this list do not alter Centers for Medicare and Medicaid Services reporting requirements or the Colorado State Board of Health requirements for reporting communicable diseases and conditions.  Annual NHSN Data Freeze April 15, 2026 The reporting deadline for NHSN healthcare-associated infections (HAI) data is April 15, 2026. Please be sure to finish all HAI data reporting by April 15th to be included in the annual report. The HAI/AR Program pulls reportable HAI data from the National Healthcare Safety Network (NHSN) on April 16 for publication in the Annual Report on September 15. Currently, the committee is requesting applicants for the following two positions: Physician, and Representative from a health insurer The physician must be board-certified or board-eligible physician, licensed in the state of Colorado, affiliated with a Colorado hospital or medical school, an active member of a national organization specializing in health-care epidemiology or infection control, and have demonstrated an interest and expertise in health facility infection control.  Applicants must be a resident of Colorado and must be able to contribute objectively to committee meetings from the perspective of the position. Healthcare-associated infections (HAI) reporting requirements and the HAI Annual Report – June 2025

 

Healthcare-associated infections (HAI) reporting requirements and the HAI Annual Report – June 2025 All separately licensed healthcare facilities in Colorado must report healthcare-associated infections (HAIs) to the Colorado Department of Public Health and Environment (CDPHE) through the National Healthcare Safety Network (NHSN) as required in Colorado Revised Statutes (C.R.S) 25-3-601-607. Reporting requirements are determined by the HAI Advisory Committee and CDPHE and differ by facility type and reporting location. Facility types include: Acute care hospitals,  Ambulatory surgery centers,  Critical access hospitals,  Inpatient rehabilitation facilities,  Long-term acute care hospitals,  Outpatient hemodialysis centers, and  Pediatric hospitals. Colorado HAI reporting requirements and deadlines can be found on the HAI/AR Program website. In order to meet the reporting requirements, healthcare facilities must include the reportable conditions in their NHSN reporting plan AND confer rights to CDPHE by joining the appropriate CDPHE reporting group in NHSN.  Separately licensed healthcare facilities in separate locations should report separately, even if they share a CMS certification number (CCN). This is consistent with NHSN guidance for facilities that share a CCN. The Centers for Disease Control and Prevention (CDC) will aggregate the data from all applicable NHSN Org IDs and will send data to Centers for Medicare and Medicaid Services (CMS) under the single CCN for CMS reporting purposes. Additionally, healthcare facilities that do not have a CCN are able to use NHSN to fulfill Colorado reporting requirements. The HAI/AR Program in the CDPHE Communicable Disease Branch distributes a survey to healthcare facilities annually in order to determine HAI reporting eligibility and status of reporting. HAI reporting requirements are verified at the time of license renewal as well.  In April 2025, C.R.S. 25-3-603 was amended such that the publication date for HAI Annual Report was changed from July 15th to September 15th beginning in 2025. The timeline for reporting HAI reportable conditions to CDPHE has not changed. Healthcare facilities are responsible for the quality and accuracy of their data reported to CDPHE through NHSN for representation in the HAI Annual Report. NHSN offers training for HAI reporting on the NHSN website. Currently, the committee is requesting applicants for the following positions: Medical statistician with an advanced degree in such specialty OR clinical microbiologist with an advanced degree in such specialty, Representative from a health insurer. Applicants must be a resident of Colorado and must be able to contribute objectively to committee meetings from the perspective of the position. All interested and qualified individuals should apply. This position will be appointed by the Executive Director of CDPHE for a term of three years. The HAI Advisory Committee was established under Colorado Revised Statute (C.R.S.) 25-3-602 to provide oversight to CDPHE for the collection and public reporting of data on HAI infection rates from health facilities. The committee may also advise CDPHE on other HAI-related activities. The committee typically meets seven to ten times each year on the fourth Tuesday of the month from 2 to 4 p.m. CDPHE facilitates committee meetings using video conferencing. There are 11 members who represent a variety of stakeholders. Membership is voluntary and qualifications are outlined in statute. Participation on the HAI Advisory Committee is a great way to learn about HAIs and play an important role in state public health! We appreciate a diversity of input. CDC call for cases: serious adverse events after ceftriaxone administration

 

To: Prescribers, Infection Preventionists, Pharmacists, Facility Administrators From: Healthcare-Associated Infections/Antimicrobial Resistance Program, Colorado Department of Public Health and Environment The Centers for Disease Control and Prevention (CDC), in collaboration with state and local health departments, is investigating reports of serious adverse events, including deaths, following receipt of injectable ceftriaxone. To date, events have not been associated with a single product manufacturer or lot and a definitive causal link to ceftriaxone has not been established. A public health investigation is underway to identify and characterize serious adverse events associated with ceftriaxone exposure. CDC is requesting reports of serious adverse events following the administration of ceftriaxone to assist with the ongoing investigation. Please report to CDC adverse events that meet the following criteria, occurring after September 1, 2024: 1. Occurred within 6 hours after receipt of injectable* ceftriaxone in a non-ICU setting, and 2. Resulted in death or required cardiopulmonary resuscitation**, and 3. Not attributed by the treating provider(s) to a cause other than ceftriaxone administration***. *including both intramuscular and intravenous routes of administration **cardiopulmonary resuscitation defined as the use of chest compressions and mechanical ventilation or provision of rescue breaths to maintain circulatory flow and oxygenation during cardiac arrest ***such as known infection, other underlying medical condition, or exposure to a medication or medical product other than ceftriaxone Please make reports to CDC’s Division of Healthcare Quality Promotion at HAIoutbreak@cdc.gov.
