New Nurse Staffing Plan Annual Report Submission Process

 

Dear Hospital Licensee,  An updated process has been implemented in order to streamline and hopefully alleviate difficulties for the required nurse staffing plan annual report submission. The required form can be found with the documents and instructions needed to complete the facility's license renewal application. Additionally, the form can be found on the CDPHE website.  In order to complete this process, please complete this form in its entirety. The form should be downloaded and completed in Adobe Acrobat. Do not complete the form in another software program. Once completed, provide the completed form as well as the staffing plan and other supporting documentation to cdphe_staffingplan@state.co.us. As a reminder, in accordance with the Standards for Hospitals and Health Facilities Chapter 4 - General Hospitals, as outlined in 6 CCR 1011-1, it is a mandatory requirement for hospitals to submit an annual report of their nurse staffing plan to the Colorado Department of Public Health and Environment (CDPHE).
