Off-Cycle Revalidation SNF

 

To: Nursing Home Management From: Jo Tansey Please see the attached document regarding your need to complete an off-cycle revalidation for certification. We wanted to make sure everyone was aware of this new CMS requirement to complete the 855A form to revalidate your certification. You should receive a letter from your MAC telling you to complete this form. Please do it right away so you do not lose your certification.
