From: The Colorado Department of Public Health and Environment’s Healthcare-Associated Infections/Antibiotic Resistance Program
The Colorado Department of Public Health and Environment (CDPHE) has created an interactive ArcGIS dashboard showing Colorado patient transfers between healthcare facilities. This dashboard is intended for hospital and public health use to better understand how patients move between healthcare facilities. The data used to construct this dashboard is publicly available Medicare fee-for-service claims data and the Centers for Medicare and Medicaid Services Minimum Data Set.
Understanding how patients are transferred and which facilities share patients frequently can help: - Healthcare providers and infection preventionists recognize the potential for interfacility spread of antibiotic resistance.
- Public health staff identify healthcare facilities that may treat patients impacted by a healthcare-associated outbreak.
- Hospitals develop emergency preparedness plans based on facility proximity and existing transfer patterns.
- Hospital infection preventionists, EMS staff, and emergency preparedness staff identify their frequent transfer partners to better understand where their patients come from and where they go after receiving care at their facility.
