Annual Facility Survey reminder 2024

 

To: Acute care hospitals, critical access hospitals, long-term acute care hospitals, inpatient rehabilitation facilities, ambulatory surgery centers, and outpatient dialysis facilities.   From: Healthcare-Associated Infections & Antimicrobial Resistance Program of CDPHE   The February 29 deadline for completion of the annual health facility survey has passed. If you have not completed the survey, please do so now. The survey helps us to ensure that your facility is accurately represented in the HAI Annual Report, and should only take a few minutes to complete. The HAI/AR Program pulls reportable HAI data from the National Healthcare Safety Network (NHSN) on April 16 for publication in the Annual Report on July 15. Colorado Revised Statutes 25-3-601-607 require health care facilities to report specific healthcare-associated infections (HAIs) to the Colorado Department of Public Health and Environment (CDPHE) via the National Health Safety Network (NHSN). Here is the list of reportable HAIs. Each year, CDPHE sends out a brief survey to licensed health care facilities across the state to collect information on what conditions facilities are reporting to NHSN, and to collect contact information.  Complete the annual health care facility survey as soon as possible. The survey was sent via email to Administrators at acute care hospitals, critical access hospitals, long-term acute care hospitals, inpatient rehabilitation facilities, ambulatory surgery centers, and outpatient dialysis facilities on 2/13/2024. Please forward the survey to the person responsible for NHSN reporting. The survey link is site-specific. Please do not forward the survey link to another facility.
