To: Acute care hospitals, critical access hospitals, long-term acute care hospitals, inpatient rehabilitation facilities, ambulatory surgery centers, and outpatient dialysis facilities. From: Healthcare-Associated Infections & Antimicrobial Resistance Program of CDPHE Colorado Revised Statutes 25-3-601-607 require health care facilities to report specific healthcare-associated infections (HAIs) to the Colorado Department of Public Health and Environment (CDPHE) via the National Health Safety Network (NHSN). Here is the list of reportable HAIs. Each year, CDPHE sends out a brief survey to licensed health care facilities across the state to collect information on what conditions facilities are reporting to NHSN, and to collect contact information.
Complete the annual health care facility survey by February 29, 2024. The survey was sent via email to Administrators at acute care hospitals, critical access hospitals, long-term acute care hospitals, inpatient rehabilitation facilities, ambulatory surgery centers, and outpatient dialysis facilities on 2/13/2024. Please forward the survey to the person responsible for NHSN reporting. If you did not receive the survey or have any difficulties completing the survey and you require assistance, please contact Amy Harjes at amy.harjes@state.co.us.
