Simplified Occupancy, and Designation Waiver Request

 

To: Hospital Providers   From: The Health Facilities and Emergency Medical Services Division at CDPHE   With the recent increase in hospitalizations and capacity concerns, HFEMSD has simplified the processing and tracking of waiver requests for related submissions. The new process will allow hospitals to submit a coordinated request that is sent to both HFEMSD and our partners at the Colorado Department of Public Safety (CDPS), Division of Fire Prevention and Control. The following waiver process may be utilized for requests related to occupancy, temporary shelters, and re-designation (i.e., change of use) of facility space.   If you need to request such a waiver, please use the new waiver request form.   Please be aware that additional information may be required to be submitted to CDPHE and/or CDPS and the waiver cannot be processed or approved until all required applicable information has been received.   Please email your questions to cdphe.hfd-revisits@state.co.us
