Isolation Plans Due Friday – 5/1/20 – 2 Days Remaining

To: Skilled nursing facilities, assisted living residences, intermediate care facilities, and residential care facilities for persons with developmental disabilities (commonly referred to as group homes) Facility isolation plans are due on Friday, May 1, 2020. Submit all isolation plans to COVID-19FacilityIsolationPlan@state.co.us using the Isolation Plan Template. For a successful submission, a facility must submit 1 completed isolation plan template for each facility to the email address listed above.    Failure to comply with Public Health Order 20-20 is subject to the penalties contained in section 25-1-114, C.R.S., including a fine of up to one thousand (1,000) dollars and imprisonment in the county jail for up to one year.
