The attached Public Health Order is in effect for all skilled nursing facilities, assisted living residences and intermediate care facilities. Facility access restrictions and mandatory entry exclusions are identified. Please follow the Public Health Order when limiting access, restricting visitors or screening individuals coming to your facilities, as you work with us stop the spread of COVID-19.
