This system will be replacing the Health Facilities Portal that you currently use to submit license applications, occurrence reports, and plans of correction for the surveys you receive.
Our fourth and final webinar session will introduce you to the Licensing functions, as well as train you how to:
- Submit an application for an initial, renewal, or change of license
- Pay for your license
- Upload your documents

Please note that COHFI is the official medium for business communication between the Department and licensed and/or Medicare/Medicaid certified health care entities. Health care entities should continue to monitor their email accounts routinely.
