COVID-19 Vaccine Information and COHFI/EMResource Assistance

 

To: Residential care facilities (Nursing Homes, Assisted Living Facilities, Group Homes, and Intermediate Care Facilities)

The CDPHE Residential Care Strike Team is reaching out to long-term care facilities to provide support to those that may need additional vaccine clinics to provide first, second, and/or booster doses for residents and staff. This outreach is also focused on providing Juvare/EMResource and Colorado Health Facilities Interactive (COHFI) reporting assistance, as necessary.  
 
We are working diligently to support your facilities so that you have the resources and knowledge needed to protect and keep your communities safe. 
 
Booster information:
The Society for Post-Acute and Long-Term Care Medicine (AMDA) is a community of more than 50,000 medical directors, physicians, nurse practitioners, physician assistants, and other practitioners working in the various post-acute and long-term care (PALTC) settings. 
 
As a way to assist the greater long-term care community, we collaborated with AMDA to provide the following COVID-19 vaccine booster materials:
 
 
For more information, visit the Strike Team Vaccination page. All three authorized COVID-19 vaccines (Pfizer, Moderna, and Johnson & Johnson) are safe, effective, and free. You should get a booster if you:
  • Are 16 or older and received your second dose of Pfizer at least six months ago.
  • Are 18 or older and received your second dose of Moderna or Pfizer at least six months ago.
  • Are 18 or older and received the Johnson & Johnson vaccine at least two months ago.

Long Term Care Facility call:
Please join us Wednesday, Dec. 15, for the next Long Term Care Facility call. This week’s call will focus on visitation and communal dining and group activities, as well as an overview of COHFI and EMResource reporting. 
 
After registering, you will receive a confirmation email with information about joining the webinar. If you have previously registered, you can use the same link for future LTC calls. Please use your facility- or agency-affiliated email address for easier verification.
