Update: Forthcoming COVID-19 Vaccination Waiver Process

 

To: All Licensed Health Facilities   From: Health Facilities Education and Quality Branch   Greetings All Licensed Health Facilities,   Per the Colorado Board of Health’s 8/30/21 emergency rulemaking, please be aware that the Health Facilities and Emergency Medical Services Division is currently developing both a unique form and process with regard to waiver submission for those facilities unable to meet the 100% staff vaccination rate due to religious exemption.   What you should know: At this time, Do Not use the currently available waiver form. On 9/17/21 you will receive a follow-up provider email which will contain the specific form, email address, and Frequently Asked Questions document for submitting this specific waiver. Waivers submitted prior to 9/17/21 for this specific rule will not be accepted. Waivers will only be required for a less than 100% vaccination rate due to religious exemption (not medical).   Thank you greatly in advance for your patience as we develop this process, and please feel free to submit any additional questions to the Residential Care Strike Team (residentialcarestriketeam@state.co.us) about the implementation of the rule.
