The Nursing Home Innovation Grant Board is posting a Call for Proposal and Grant Application For Cycle 8.
The Cycle 8 grant period will start July 2016.
The Call for Proposal is posted on the CDPHE website, CHCA's website and the website for HCPF.
All applications need to be returned to Jamice Ormond at jamice.t.ormond@state.co.us no later than April 29, 2016
