HCBS Settings Final Rule – PTP training announcement

To: Provider Directors From: HFEMSD The Department of Health Care Policy and Financing (HCPF) sent an Informational Memo to inform providers of non-residential and children’s residential Home and Community-Based Services (HCBS) of the upcoming rollout of the web-based platform on which to complete Provider Transition Plans (PTPs) and of upcoming trainings on how to complete their PTPs.
