The Department of Health Care Policy and Financing (HCPF) sent an Informational Memo to inform providers of adult residential Home and Community-Based (HCBS) waiver services of upcoming training on how to complete the web-based Provider Transition Plan (PTP). Please review the attached Informational Memo for more detail, and to access the link to register for one of two webinars offered on September 10, 2018 and September 14, 2018.
