To: All Providers The Health Facilities and Emergency Medical Services Division receives numerous requests each year for information on the most frequently cited deficiencies across survey programs. Recognizing the need for this data, the Health Facility Education and Quality Branch enlisted the help of the division’s Data Section. Staff analyzed data from various databases and organized the deficiencies by survey type. The Training Section staff then researched these deficiencies to provide additional context.
