2023 Colorado Hospital Stewardship Communication Survey

 

To: Hospitals infection preventionist and antimicrobial stewardship contact   From: CDPHE   The Healthcare-Associated Infections and Antimicrobial Resistance (HAI/AR) program within the Communicable Disease Branch at the Colorado Department of Public Health and Environment (CDPHE) wants your feedback regarding antimicrobial stewardship.   We would like to know which topics in stewardship and forms of communication would best support the development and advancement of your antimicrobial stewardship program. We will also use this information to determine how to best support facilities with the NHSN AUR module requirement of the Public Health and Clinical Data Exchange Objective of the CMS PI program.   The survey should take you no more than five minutes to complete.
