TO: Acute Care Hospital, Critical Access Hospital, Long Term Acute Care Hospital, Inpatient Rehabilitation Facility, and Ambulatory Surgery Center FROM: Colorado Department of Public Health & Environment (CDPHE) The Healthcare-Associated Infections and Antimicrobial Resistance (HAI/AR) Program within the Communicable Diseases Branch at the Colorado Department of Public Health and Environment conducts an annual survey to assess conditions reportable to the National Healthcare Safety Network (NHSN) and to be included in the HAI Annual Report. Certain HAI conditions are reportable in Colorado under TITLE 25, ARTICLE 3, PART 6 of the COLORADO STATUTES GOVERNING HEALTH FACILITY-ACQUIRED INFECTIONS DISCLOSURE and can be found here: https://drive.google.com/file/d/17VTyqoP2YSlQVUE7vdwmFOP-Woxq05JJ/view. The survey was sent on January 26, 2023 to healthcare facilities listed in Colorado Health Facilities Interactive (COHFI) system as acute care hospital, critical access hospital, long-term acute care hospital, inpatient rehabilitation facilities, and ambulatory surgery center. The email with the survey link was sent directly to the facility administrator listed in the COHFI system and/or the person responsible for infection prevention and/or NHSN reporting at your facility. The survey should take no more than 10 minutes to complete and the deadline has been extended to Friday, 2/24/2023.
