REQUEST: 2022 Colorado HAI Annual Survey Due on 1/28

 

TO: ACHs, LTACHs, IRFs, CAHs, SNFs and Children Hospitals in Colorado   FROM: Colorado Department of Public Health & Environment (CDPHE), Healthcare Associated Infections & Antimicrobial Resistance Program   The Healthcare-Associated Infections and Antimicrobial Resistance (HAI/AR) Program within the Communicable Diseases Branch at the Colorado Department of Public Health and Environment conducts an annual survey to assess conditions reportable to the National Healthcare Safety Network (NHSN) and to be included in the HAI Annual Report.   Certain HAI conditions are reportable in Colorado under TITLE 25, ARTICLE 3, PART 6 of the COLORADO STATUTES GOVERNING HEALTH FACILITY-ACQUIRED INFECTIONS DISCLOSURE and can be found here: https://cdphe.colorado.gov/hai-professionals.   We emailed the 2022 HAI survey directly to the person responsible for infection prevention and/or NHSN reporting at your facility listed in the 2021 HAI Annual survey that was completed last year. Your facility is required to complete the survey no later than 1/28/2022.   If the person responsible for infection prevention and/or NHSN reporting at your facility did not receive the link to the survey, the appropriate contact information has changed, or if there are questions about how to complete the survey, please contact Lynda Saignaphone at Lynda.Saignaphone@state.co.us for assistance.
