Nursing Home Admission Guidance Update

The Colorado Department of Public Health and Environment updated the all-in-one mitigation guidance for nursing homes on Jan. 25. Among the changes is guidance that allows unvaccinated residents to be admitted while a facility is in an outbreak. Guidance is as follows (located on page 28):
 
Create a Plan for Managing New Admissions and Readmissions
 
  • Residents with confirmed SARS-CoV-2 infection who have not met criteria to discontinue Transmission-Based Precautions should be placed in the designated COVID-19 care unit, regardless of vaccination status.
  • Newly admitted or readmitted residents who are not fully vaccinated (even if they test negative upon admission) should be placed in a private room for 14-days and cared for by HCPs wearing full PPE as described in the source control and physical distancing table. If a facility is experiencing an outbreak potential admissions that are not fully vaccinated should receive a copy of the CDPHE document “Unvaccinated RCF admission during a COVID-19 outbreak” prior to their admission, informing them of their increased risk of COVID-19.
  • Facilities located in counties with low community transmission might elect to use a risk-based approach for determining which unvaccinated residents require quarantine upon admission. Decisions should be based on whether the resident had close contact with someone with SARS-CoV-2 infection while outside the facility and if there was consistent adherence to IPC practices in healthcare settings, during transportation, and in the community prior to admission.
  • Fully vaccinated residents and residents who had a SARS-CoV-2 infection in the last 90 days do not need to be placed in quarantine.
