BHE-Confirmation of Physical Addresses Not Used for Service Delivery

To: Community Mental Health Centers, Community Mental Health Clinics, Acute Treatment Units, Crisis Stabilization Units   From: Jane Flournoy     The Health Facilities and Emergency Medical Services Division (HFEMSD) of the Colorado Department of Public Health and Environment (CDPHE) has been working to ensure any and all physical locations associated with licensed community mental health centers, community mental health clinics, acute treatment units and crisis stabilization units are included in the transition to the Behavioral Health Entity (BHE) license. Additionally, HFEMSD is seeking to clarify which physical locations associated with licensed community mental health centers, community mental health clinics, acute treatment units and crisis stabilization units transitioning to the Behavioral Health Entity (BHE) license are not intended for use for on-site service delivery. As such, please clarify any and all physical addresses associated with your currently licensed CMHC, CMHc, ATU and/or CSU that you do not intend to have included in the list of physical locations where on-site services will be offered in this google form.
