Reminder: Additional Dose/Booster vaccine clinic surveys due, CDPHE representatives will be in contact

To: Residential care facilities (nursing homes, assisted living facilities, group homes, and intermediate care facilities)

From: CDPHE Residential Care Strike Team

CDPHE is offering to coordinate a COVID-19 additional dose/booster dose vaccine clinic at your facility for all residents and staff members. This opportunity will allow those who are moderately to severely immunocompromised (and already vaccinated with two doses of an mRNA COVID-19 vaccine) to receive an additional dose of the Pfizer or Moderna mRNA COVID-19 vaccine. Additionally, those who are not yet fully vaccinated will be offered a first or second dose of the Pfizer or Moderna vaccine.

All facilities should have received the LTCF Additional Dose/Booster Survey and completed it by August 25. Thank you for completing this for our planning purposes. If you have not completed the survey, please do so immediately.

A CDPHE representative will be contacting the person who completed the survey for additional information to begin the planning and vaccine clinic scheduling process. This correspondence from CDPHE representatives will occur whether your facility has indicated a need for assistance or not. You will also receive contact if you did not complete the survey.

Make every effort to connect with the CDPHE representative when contacted and provide the requested information. We do not want these vaccination clinics to be delayed or unable to be scheduled due to lack of communication.

CDPHE can offer vaccine, storage/handling, and staff to the facility as well as administrative support to document all of the COVID-19 vaccines administered in the Colorado Immunization Information System (CIIS), as required.

To prepare for CDPHE contact:

  • Discuss additional and booster vaccine doses with residents, staff, and resident decision makers to determine how many persons (residents and staff) at your facility are eligible per CDC’s definition of immunocompromised for a third dose now and those who may be eligible for a booster dose soon.

Total number of doses needed for:

  • Immunocompromised staff/residents who need a third dose.
  • Non-immunocompromised residents who need a first or second dose.
  • Non-immunocompromised staff who need a first or second dose.
  • Pfizer or Moderna.

This should include all residents and staff who have:

  • Received a second dose of either Pfizer or Moderna six to eight months ago.
  • Immunocompromised and require an additional dose of either Pfizer or Moderna (>28 days after second dose).
  • Not received their first or second dose of the primary mRNA vaccine series.

  • Determine a date(s) at least six to eight months after the majority of your residents and staff received a second dose of Pfizer or Moderna COVID-19 vaccine that would be ideal for the facility to host a third dose clinic between September 20 and October 11 (first, second, third choice). These people should be eligible for a third dose per CDC guidelines.
  • Be prepared to share the name of your vaccine provider and tentative vaccine clinic date(s) with the CDPHE representative.
  • When contacted by CDPHE, please describe the level of support your facility requires.
  • Levels of support CDPHE can offer:
  • An entire CDPHE team that provides doses and supplies needed.
  • Qualified vaccinators to assist our staff.
  • CIIS data entry.
  • Vaccine doses and supplies:
  • Vaccine manufacturer and the number of doses needed
  • Number of Pfizer doses
  • Number of Moderna doses
