To: Hospitals   From: Governor’s Office in partnership with CDPHE COVID-19 Vaccine Program Team   Body:   Dear Primary Care Provider,    As a trusted source of information for your patients, you play an important role in the effort to increase COVID-19 vaccination uptake in our state. The Colorado Department of Public Health and Environment (CDPHE) is inviting your primary care practice to enroll in the COVID-19 Vaccination Program as part of its efforts to quickly mobilize additional COVID-19 vaccine providers across the state and to increase access to COVID-19 vaccination for all Coloradans. As we transition from a mass vaccination model to more provider-focused efforts, we need your help now more than ever.   To prepare your practice for participation in the COVID vaccine program and to answer your questions, CDPHE is hosting a webinar for primary care providers and their local partners in collaboration with the Colorado Department of Health Care Policy and Financing (HCPF) at 4:00 pm on Monday, June 28,2021. The webinar will provide information to Primary Care providers and vaccine managers about the COVID-19 vaccine program and helpful resources. Moreover, HCPF staff will discuss reimbursement and incentive options followed by CDPHE State Lab staff presenting a suite of options for tackling COVID-19 within the Primary Care setting. The webinar will close with an additional 15-minute Q & A session.   In the interim, we’re also sharing our Primary Care Provider Communications Toolkit which includes ready-to-use email, text, and social media messages you can use now to encourage your patients to get vaccinated or to set up an appointment once your clinic becomes a COVID-19 vaccine provider.
