HCPF memo and informed consent template for rights modifications in HCBS settings

The Department of Health Care Policy and Financing (HCPF) sent an Operational Memo to inform Home and Community-Based Services providers and case management agencies of the required standardized template for obtaining individuals’ informed consent to a rights modification.
