2021 Colorado Long Term Acute Care Hospital Survey

TO: Long Term Acute Care Hospitals in Colorado FROM: Colorado Department of Public Health & Environment (CDPHE) The Healthcare-Associated Infections and Antimicrobial Resistance (HAI/AR) Program within the Communicable Diseases Branch at the Colorado Department of Public Health and Environment conducts an annual survey to assess conditions reportable to the National Healthcare Safety Network (NHSN) and to be included in the HAI Annual Report. Certain HAI conditions are reportable in Colorado under TITLE 25, ARTICLE 3, PART 6 of the COLORADO STATUTES GOVERNING HEALTH FACILITY-ACQUIRED INFECTIONS DISCLOSURE and can be found here: https://cdphe.colorado.gov/hai-professionals. The person responsible for infection prevention and/or NHSN reporting at your facility should complete the survey no later than 2/5/2021.
