Request for Nursing Home Data for Situational Awareness

To: Skilled Nursing Facilities/Nursing Facilities From: Greg Schlosser, Branch Chief, Health Facility Education and Quality To gain a better situational awareness understanding of COVID-19’s impact on your nursing facility as well as nursing facilities statewide, the State Emergency Operations Center (SEOC) in collaboration with CDPHE’s Health Facilities and Emergency Medical Services Division (HFEMSD) is requesting information from you to assist in local and state response efforts. To gather this information, the SEOC as well as state/local public health are using a tool called EMResource. We are asking that you request access for account creation using the form link below. Once you have requested access, you will receive login information to add your facility’s information (i.e. bed availability, PPE supply, etc.) into this tool to help the state gain more knowledge on how COVID-19 is affecting your facility.  To assist you in providing this information in EMResource, we have created both a new users training document and a short three-minute video to walk you through the process. Once you understand the process, please request access to EMResource by completing this form. If for any reason you cannot access this GoogleForm, please email cdphe_epr_sit@state.co.us . We have also designated a primary HFEMD contact once you have requested/received your EMResource account login information.
