Required Isolation Plan: Needed Response by 5/1/20

Effective April 21, 2020 Public Health Order 20-20 has been amended to include additional measures to slow the spread of novel coronavirus 2019 (COVID-19). The Public Health Order can be located here:

Skilled nursing facilities, assisted living residences, intermediate care facilities, and residential care facilities for persons with developmental disabilities are required to submit an isolation plan to the Health Facilities Division by May 1, 2020. Facilities will submit those plans to: COVID-19FacilityIsolationPlan@state.co.us. To assist with plan development, a template has been developed and can be located here: Isolation Plan Template

A new web page has also been created to provide direction and guidance for long term care and residential facilities. To view this page, which contains the public health order, facility guidance, and other resources visit https://covid19.colorado.gov/ltcf.

Failure to comply with this order is subject to the penalties contained in section 25-1-114, C.R.S., including a fine of up to one thousand (1,000) dollars and imprisonment in the county jail for up to one year.

For questions about how to submit your isolation plan, please contact (303) 692-2836.
