Recruiting QMAP Instructors for Fiscal Year 2016 (July 1, 2015-June 30, 2016)

Beginning March 16, 2015, the Qualified Medication Administration Program (QMAP) will accept applications from interested instructors for the 2016 Fiscal Year, which begins July 1, 2015 and ends June 30, 2016. Completed applications must be received no later than April 30, 2015.

Qualified medication administration personnel (QMAP) classes teach unlicensed staff members to safely administer medications in certain settings where it is authorized by law, primarily assisted living residences, residential child care facilities and adult day care programs. These facilities are regulated by the Colorado Department of Public Health & Environment (CDPHE).

Instructor Requirements And Application Instructions:


1. Instructors must be a licensed professional: LPN, RN, DR, PA and must be in good standing with the Colorado Department of Regulatory Agencies (DORA).
2. Instructors and vendors must apply annually. Previous/current instructors/vendors will NOT automatically renew or be pre-approved.
3. All interested instructors must submit a letter of intent.
4. Once the letter of intent is received the application packet will be sent via email only. The email will contain an Instructor packet and a vendor packet.
5. All completed application packets are due to CDPHE via US Mail no later than April 30, 2015. Instructor applications need to be submitted with the associated vendor packet. Incomplete packets will not be considered.
6. There is a 4 tier approval process for instructors:
- Tier 1 approval consists of receiving a completed application packet.
- Tier 2 approval requires new instructor to successfully monitor a current instructor training no later than May 30, 2015.
- Tier 3 approval requires you to register for and attend one of the mandatory instructor training sessions being held in June, 2015.
- Tier 4 is the final approval from the CDPHE's purchasing group.
7. Once finalized, you will receive:
- A letter of approval
- Purchase order number
- Medication administration curriculum and Instructor packet
8. FY16 Instructors ARE NOT allowed to schedule or conduct any classes until Tier 4 approval is granted by CDPHE and you receive a purchase order with the official approval letter. FY16 approved instructors will not be allowed to be conducted prior to July 1, 2015.

Please note: Current instructors for FY15 will be allowed to conduct classes through the current contract term which ends June 30, 2015.

Definitions of "instructor" and "vendor"
There are differences between instructors and vendors.
- "Instructors" are individuals who teach the QMAP class
- "Vendors" can be businesses or an individual who is responsible for 1099 tax information
- An individual instructor can be their own vendor.
- Business vendors can have multiple instructors.

How to determine who should be listed as your vendor:
- If you are currently employed and will be conducting QMAP classes as part of your job duties for that employer, then the employing business will be considered the vendor.
- If you are interested in conducting QMAP classes on your own without employer involvement then you would be considered your own vendor.
