Documentation for desk revisits submittal instructions

 

To: Facility/Agency Administrators From: HFEMSD, Quality Services Support Team Dear provider, As a reminder when submitting documentation for desk revisits, please follow the below guidelines: Ensure proper labeling for each deficiency/tag. Submit all documentation through the COHFI Survey Tracking application by selecting the “Documents” button on the main survey screen. Click on the “Choose Files" button, select the files you wish to upload, then select the "Upload Files" button, and the uploaded documentation will be sent to HFEMS for review. Ensure that the documentation for each deficiency is a separate attachment. The title of the attachment will be the deficiency/tag number. If multiple attachments are necessary for one deficiency, continue to use the deficiency number but include a number or letter in parenthesis to identify multiple attachments.
