To: All licensed and state-certified providers From: HFEMSD The Division IDR Policy has been amended to include the process for review of focused surveys conducted pursuant to 6 CCR 1011-1, Chapter 2, Rule 12.4, “Reporting Requirements” for COVID-19 vaccination rate. The IDR instructions that accompany the deficiency lists also have been updated to track the process set forth in the revised IDR policy.  Elaine Sabyan
